Medication Regimen Reviews:
Classic Care Pharmacists, many of which are Certified Geriatric Pharmacists, conduct routine medication regimen reviews, assessing each review for medication appropriateness. The pharmacist will provide written recommendations for the nurse and physician to review, if needed.

Committee Representation:
Classic Care Pharmacy representatives are active members of various committees within the home. As Classic Care specializes in senior-care pharmacy services for a wide range of homes, we can often provide a great deal of insight. The following is a list of committees in which Classic Care representatives can participate upon request:

Medication Audits:
Classic Care Pharmacy performs routine medication audits of medication profiles and order processing techniques within the home. These audits are detailed and can help the home's Director of Care identify areas, which potentially need improvement. The pharmacy representative performs them randomly and upon request. The ultimate intent of this process is to ensure correctness and resident safety. The following is a brief outline of areas often audited:

Physician's Orders:
• Ensuring all orders are signed by the physician and double-checked by nursing

Physician's Medication Reviews:
• Verification that all orders and necessary signatures are present/correct

Admission/Readmission Orders:
• Ensuring all information is complete/correct and signatures are present

Drug Record Book:
• All medications are properly documented as ordered/received

MAR/TAR Sheet:
• Transcription, administration, and order change/discontinuation documentation is correct

PRN Charting:
• Ensuring administered doses are documented correctly, paying particular attention to reason for use and outcome charting

Dispensed Medication:
• Ensuring that all medications are dispensed as ordered and verifying changes/discontinuations are processed correctly

General Systems:
• The following areas are audited for best practice technique:

  • Medication Carts
  • Narcotic Medications
  • Medication Refrigerator
  • Emergency Medication/Stat Box
  • Medication Disposal

Upon completion of the audit, the Classic Care representative:
• Discusses the results with the Director of Care
• Aids in developing an action plan, if warranted
• Provides a copy for the home's records
